Online Application for Cleanup Reimbursement
The purpose of Clean Up reimbursement program is to assist Utahns who have experienced a loss due to suicide. The program may reimburse you for the disaster cleanup and biohazard services needed to have homes cleaned after losing someone close to suicide. This is a very difficult time and we want to make this process as easy as possible.
You will complete an application, which will include your personal information and your consent to use this information for the reimbursement clean up program. Your information will not be used for any additional purpose. Please note that if you do not consent, we cannot proceed with financial assistance for reimbursement of the cleanup services.
Coverage limited to no more than $4,900. You must use a professional biohazard cleanup company. If you do not have the funds to pay the company up front or the insurance deductible, please contact us directly at [email protected].
